What personal data do we collect?

We collect personal data when you register with us, make an enquiry or use our website.

When you register with us, we collect information such as your title, full name, postal address, email address and telephone number. We may ask for other contact information, and we will record any other information you provide us with.

Marketing preferences, such as whether you opt in or opt out or receiving communication from us.

Information about your use of our website, such as how you came to our website, which pages you visit and how long you remain on each page. To collect this data, we use cookies.

How do we use and share personal data?

We use personal data to provide the service you request, namely healthcare to your pet(s). This allows us to perform our contract with yourself, which is to provide health care to your pet(s).

We use personal data to contact you about your pet’s health, such as vaccination reminders and appointment confirmation.

Referral practices

When we refer an animal to a specialist or a hospital we need to share relevant data to arrange appointments, treatments and services. The information which we share can be seen below:

 Laboratories and crematorium

When we send and receive results for tests, or send your pet to the crematorium we share relevant data. The information which we share can be seen below:

Insurance companies

When we submit insurance claims or pre-authorisations on your behalf we share your personal data with the relevant insurance company. The information which we share can be seen below:

  • Address
  • Telephone number (home and/or mobile)
  • Email address

Microchip companies

When we register a microchip which we have implanted into your pet, we will share your personal data with the microchip company. The information which we share can be seen below:

  • Telephone number (home and/or mobile)
  • Email address

 Debt collection agencies

Where necessary, we might need to share personal data with debt collection agencies, if you fall behind with payments. The information which we share can be seen below:

How do we protect your data?

We endeavour to protect your information by keeping your information up-to-date and accurate. Please note that, to do this, we require you to tell us if any of your details such as your name or address change.

We also have in place strict security procedures for the storage and disclosure of your information to prevent unauthorised access.

What do we NOT do with your data?

We do not sell your personal data, we do not rent your personal data.

We do not make automated decisions based on your data.

 How long do we keep personal data?

We will keep personal data for as long as is necessary to provide our service to you, and provide healthcare and treatment to your pet(s). We also keep personal data for other legitimate business purposes, such as complying with our legal obligations and resolving disputes. Retention times will vary based on this.

Your rights

You have a right to request to view the personal data held by us at any time. We ask that you request this in writing either by email to info@qpvs.co.uk or by post to Queens Park Veterinary Surgery, 74-76 Chorley New Road, Bolton, BL1 4BY. You have a right to retract your consent for us to contact you at any time, however this may mean we are unable to remind your of your pet’s vaccination status or appointments. If you wish to retract your consent for us to contact you, please write to us by email to info@qpvs.co.uk or by post to Queens Park Veterinary Surgery, 74-76 Chorley New Road, Bolton, BL1 4BY.

From time to time we may ask you to update your information to ensure that we are only holding correct and current personal data. You will be asked to refresh your consent for us to store and use this data.

Terms and Conditions

 

  1. Client Confidentiality

All information relating to both client and patient is held in confidence and in line with the Data Protection Acts and will not be disclosed unless express permission is granted from you, the client, or as part of a legal process.

  1. Ownership of Records

Medical records and similar documents (eg radiographs or ultrasound scans) are the property of Queens Park Veterinary Surgery. Copies of these records may be passed on to a referral veterinary surgeon or insurance company, with your permission.

  1. Payment of fees

All fees must be paid for at the time of treatment, unless an insurance claim has been pre-authorisation and agreed.
VAT is charged at the current rate on all fees. We will provide a detailed invoice for all fees, consultations and surgeries. We will provide an estimate of fees for all work undertaken, please be aware that this will be an estimate but will be as accurate as possible. Prices are available on our website or by request.

  1. Methods of payment

All fees are due at the time of treatment. You may settle your account by cash, debit/credit card or by direct insurance claim with prior agreement. We will provide our bank details upon request for BACS payments, but this must be pre-authorised by the Practice Manager.

We do not provide credit accounts.

  1. Failure to make payment

Failure to make payment within 14 days of treatment may incur additional charges.

  1. Complaints
